ROLE
Lead Communications and Project Manager for the Medicare Chronic Kidney Disease Program (CKD). I worked with clinicians and was responsible for the content for newsletters and presentations. Project collateral was featured at a poster session at the CMS Quality Conference in Baltimore, MD.
ABOUT THE PROJECT
CKD Project began in 2007 as a multi-state (10 states) pilot project.

Engaged physicians to make system-level changes using quality improvement principles and concepts.

Conducted site visits to provide expert guidance and technical assistance on clinical measures to providers and stakeholders.

Developed and implemented strategies and oversaw an integrated marketing campaign to target populations with health disparities for patient engagement.

Planned and implemented communications activities for physicians, hospitals, and a statewide coalition. Also planned and executed strategies for recruitment, engagement, and retention of providers, stakeholders, community health workers, and Medicare beneficiaries.

Planned, directed, and implemented communications activities, including webinars, learning and action networks, print collateral, website, social marketing, and eNewsletters for physicians, hospitals, and statewide coalition (open rate 71%). Ensured that key, strategic messages were communicated effectively to target audiences.

Planned and executed strategies for recruitment, engagement, and retention of providers, stakeholders, community health workers, and Medicare beneficiaries.

Layout and design marketing, communications collateral and make 508 compliant.

Conducted focus groups and other market research activities, including surveys, text, and webinar polls.
Key Achievements:

86% physician engagement rate for the CKD QI project.

Developed the electronic eGFR Tracking and Trending Tool to assist providers in maintaining system-level changes and presented as best practice at CMS Quality Conference.

Conceptualized a database for tracking and capturing project quality improvement data including developing a data abstraction tool for physician practices.
