ROLE 

Designed/Administered/Subject Matter Expert for the Medicare Everyone with Diabetes Counts (EDC), a diabetes self-management program targeting African Americans and Hispanics to reduce health disparities.

 

ABOUT THE PROJECT

EDC began in 2006 as a single-state pilot project in Florida. Based on key findings from this pilot, the project was expanded between August 2008 and July 2012 to include Maryland, Washington, D.C., Louisiana, Georgia, New York, Mississippi, the U.S. Virgin Islands, and specific regions in Texas. 

I became a Senior Trainer with the University of Illinois at Chicago for the Diabetes Education Empowerment Program (DEEP), training over 200 community health workers as peer educators and lead trainers in Arizona, California, Florida, Ohio, and the US Virgin Islands.

 

In August 2014, EDC was launched nationwide. The HSAG QIN-QIO exceeded CMS measure targets. With leadership and oversight of the US Virgin Islands QIO, my Team and I met and exceeded CMS recruitment targets and eventually patient improvement outcomes. The QIN-QIO exceeded the 16,000 participant education goal and served over 25,000 Medicare beneficiaries. Below are samples of presentations, reports, and collateral that I wrote, designed, and created. I formulated strategies for distribution. (Note:  EDC logo was not created by myself.)
